Daily Living and Life Skills – 12 Session Capacity Building Program

This program is very close to my heart. For almost five years I have worked with people living with many different disabilities, and I have seen their daily struggles and how difficult it can be to access the services they need to live with dignity and enjoy the life they dream of.

If you’re living with disability and feel exhausted, disorganised and overwhelmed by health advice, this 12-session program is designed to give you structure, skills and support, without expecting you to be perfect

I developed this 12-session program to provide structured, evidence-informed support that fits within NDIS funding. My goal is to make it easier for participants and caregivers to access the mentoring, education and practical tools they need to build skills, increase independence and feel more in control of your everyday life.

Who this is for

Adults living with disability who would like support to:

  • Create or improve daily routines (sleep, meals, movement, rest)

  • Plan and prepare simple meals that fit their health needs and abilities

  • Understand their health conditions and the advice from doctors in plain language

  • Manage fatigue, pain and energy when planning their day

  • Feel more confident going to and following up on health and other appointments

  • Join in more meaningful home, community, social or work-related activities

Maybe right now you feel tired, stuck, or like you keep “starting again” with routines that don’t last.
You do not need to be disciplined or perfect to begin.
We simply start from where you are and build small, realistic steps together.

What we work on together

Every person is different, but common focus areas include:

  • Meal planning and food shopping that work with your disability, budget and energy

  • Simple meal preparation and cooking skills

  • Building or adjusting daily routines so they fit your real life

  • Getting ready for appointments (what to take, what to ask)

  • Turning health advice into small, doable actions

  • Planning your day or week around your energy, fatigue and pain

  • Problem-solving challenges at home, in the community or at work/study

Sessions are practical and tailored to your disability, goals, culture and support needs.
The focus is on building skills and confidence in daily life.

You are not expected to fix everything in 12 sessions.
We focus on building a solid foundation, a few key routines and practical tools, and a kinder way of working with yourself that you can keep using after the program finishes.

How the 12-session program works

  • 12 individual 1:1 sessions, usually over 3–6 months

  • Sessions are held in your home

  • All 12 sessions are to be used within 6 months of your first session, to support momentum and real progress

Program structure:

  1. Foundation session (120 minutes)

    • Understand your current situation, supports and daily routines

    • Clarify your NDIS-linked goals and priorities

    • Agree on focus areas for the program

  2. 10 skill-building sessions (90 minutes each)

    • Short education in plain language

    • Guided practice of new skills and routines

    • Problem-solving real-life barriers (fatigue, pain, memory/thinking, environment)

    • Regular review and adjustment at your pace

  3. Final review session (around 90 minutes)

    • Review changes in your skills, routines and confidence

    • Confirm what has helped you most

    • Create a simple written plan you (and your supports) can keep using

Pace:

  • We agree on a pace that fits your energy and life

  • This might look like weekly sessions at first, then fortnightly as habits settle

  • Or mostly fortnightly sessions, with a few weekly “boost” sessions when needed

PROGRAM STRUCTURE AND PACE

Important: Sessions are practical, education-based and tailored to the participant’s disability, goals, culture and support needs. The work is focused on capacity building – developing skills for everyday life, not providing medical treatment, diagnosis or alternative therapies.

  • 1 x 120-minute foundation session

  • 11 x 90-minute follow-up sessions

  • A personalised coaching plan based on your goals, health and life context

  • Clear program goals and step-by-step actions

  • Practical tools such as simple trackers, planning templates and reflection prompts

  • Optional symptom, energy or mood tracking if helpful for you

  • Light messaging support between sessions (business hours) for brief check-ins about your agreed actions

Steps to get started

  1. Free 20-minute discovery call

    We talk about your goals, current situation and capacity.
    You can ask questions about how I work, and together we decide whether this 12-session program is the right fit for you.

  2. Foundation Session

    Before we officially move into a 12-session program, we start with a separate Foundation Session. In this session, we take time to understand your current situation, supports and daily routines, clarify your NDIS-linked goals and priorities, and check how my approach fits your needs. Together, we agree on clear focus areas for the program so we can both assess whether working together is the right next step for you.

  3. Onboarding: If we decide to work together, you:

    • Sign the coaching agreement

    • Complete a short intake form

    • Fill in simple baseline questionnaires (if relevant)

    We schedule your sessions and confirm any key information from your GP or specialist that we need to consider.

  4. Coaching and support

    We meet for your 1:1 sessions in your home.
    Between sessions, you can use light messaging for brief updates and questions about the actions we agreed on.

  5. Review and next steps

In your final sessions, we review your progress, refine your ongoing plan and discuss what comes next:

  • further sessions,

  • occasional check-ins, or

  • moving to self-guided change.

Frequently Asked Questions

Is this therapy or medical treatment?

No. This program is not medical treatment, counselling or psychotherapy.
I do not diagnose, prescribe or change medications, or replace your GP or specialists.

This is a coaching and education program focused on building skills and capacity in your day-to-day life: routines, planning, problem-solving, health literacy and self-advocacy. I encourage you to keep working with your existing health professionals.

Can I use my NDIS funding for this program?

In most cases, yes.
This program is designed to fit within NDIS Capacity Building funding:

The exact category will depend on your goals and plan. You, your Support Coordinator or Plan Manager can check your plan and we can discuss the best fit together.

Do I need a specific diagnosis to work with you?

No specific diagnosis is required.

I work with adults living with many types of disability, including physical disability, chronic illness, neurodivergence, psychosocial disability and mixed/complex conditions. What matters is that your goals relate to building skills and capacity in everyday life and that the program is a good fit for your needs.

What if I have very low energy, brain fog or bad pain days?

We plan for that.

Sessions are flexible and paced gently. On days when energy, pain or brain fog are high, we might:

  • Keep things very simple and focus on one small step

  • Use more visual supports or written summaries

  • Spend time adjusting plans so they are kinder and more realistic

You do not need to “perform” or be at your best for this to work. Part of the work is learning how to support yourself on the hard days, not just the good ones.

Can a family member or support worker join the sessions?

Yes, if you want them there.

With your consent, a family member, carer or support worker can join some or all sessions. This can help everyone understand your routines, energy and goals, and support you in a consistent way at home and in the community. You are always in charge of who is involved and what is shared.

What happens in a typical session?

Every session is tailored, but a typical session may include:

  • A brief check-in about how you have been since last time

  • Reviewing any small actions you chose to try (with no shame if they didn’t happen)

  • Exploring one area of your life where you want support (sleep, meals, appointments, routines, pacing, etc.)

  • Learning or practising one or two practical tools or strategies

  • Agreeing on one or two gentle steps to try before we meet again

You never have to do anything you are not comfortable with. We decide together what feels realistic for you.

Do we talk about weight or diets?

Only if you want to, and always with care.

My focus is on supporting you to feel safer, more nourished and more supported in your body and daily life. That usually means:

  • Simple, sustainable food routines that work for your body, disabilities and preferences

  • Building a kinder relationship with your body and your energy

  • Following any medical nutrition advice from your treating team, if relevant

I do not promote restrictive diets, “quick fix” weight-loss plans or shaming language about bodies.

What if I already have other therapists or supports?

That is completely fine.

This program can sit alongside your existing therapy, allied health or support services. Where helpful and with your written consent, I can communicate briefly with your other providers so we are all working in the same direction and supporting your NDIS goals.

What if I am nervous about starting?

That is very normal. Many people feel unsure at first.

Our first session is about getting to know you, your story and your priorities at a pace that feels safe. There is no expectation to share everything straight away. You are always welcome to ask questions, take your time, and decide whether this feels like a good fit for you.
